Practice Name: The Healing Hour, LLC
1. Agreement to Services
By accessing or using services provided by The Healing Hour, LLC (“Practice,” “we,” “us,” or “our”), you agree to be bound by these Terms of Service (“Terms”). These Terms govern your use of our mental health services.
2. Nature of Services
The Practice provides outpatient mental health therapy services. Therapy is a collaborative process and outcomes may vary by individual. Participation is voluntary, and you may withdraw from services at any time.
3. Informed Consent
By engaging in services, you acknowledge that you have reviewed and agreed to the Practice’s Consent for Services, including:
- The structure and goals of therapy
- Risks and benefits of treatment
- Your rights and responsibilities as a client
4. Appointments and Cancellations
- Clients are responsible for attending scheduled sessions on time.
- Late cancellations and missed appointments may be subject to fees.
- Policies regarding cancellations are outlined in your intake documentation.
5. Telehealth Services
Telehealth services are provided in compliance with applicable laws and regulations. By participating, you acknowledge:
- Potential risks related to technology and confidentiality
- Limitations in emergency response
- Responsibility to ensure a private and secure environment
6. Confidentiality and HIPAA Compliance
The Practice complies with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable state laws governing Protected Health Information (PHI).
Your PHI may be used or disclosed without your authorization for:
- Treatment
- Payment
- Healthcare operations
Your information may also be disclosed when required by law, including but not limited to:
- Risk of serious harm to yourself or others
- Suspected abuse or neglect
- Legal proceedings or court orders
All other uses and disclosures require your written authorization.
7. Communication
- Electronic communications (email, SMS) are not guaranteed to be secure.
- The Practice may use secure systems when appropriate.
- The Practice does not engage with clients via social media to protect confidentiality and professional boundaries.
8. Fees and Payment
- Payment is due at the time of service unless otherwise agreed in writing.
- You are responsible for all fees not covered by insurance.
- Outstanding balances may be subject to collections.
Additional fees may apply for:
- Missed or late-canceled appointments
- Administrative services (e.g., reports, letters, court-related services)
9. Insurance
If utilizing insurance benefits:
- Coverage is determined by your insurance provider
- You are responsible for deductibles, copayments, and non-covered services
- Denied claims remain your financial responsibility
10. Records and Documentation
The Practice maintains clinical records in accordance with HIPAA and applicable laws. These records are securely stored and used to ensure continuity and quality of care.
11. Termination of Services
Either party may terminate services at any time. The Practice may provide referrals or recommendations for continued care when appropriate.
12. Amendments
The Practice reserves the right to modify these Terms at any time. Updates will be posted on the website with an updated effective date.
